It’s time once again for our annual open enrollment for employee benefits. This is the time of year when eligible employees can make changes to existing benefit coverages or enroll in benefits that are not currently utilized. It has always been a priority for us to take care of our Axiom family by offering a comprehensive benefit package to all eligible employees with low out of pocket costs, and we have been able to do so again for 2019. In 2019 Axiom will continue to utilize the Aetna PPO network, a comprehensive network with more than 850,000 participating providers and 6,900 hospitals. Aetna PPO Network: http://www.aetna.com/docfind/custom/mymeritain/
This year, we will be offering TWO comprehensive health plan options for 2019, AND we will be adding additional “tiers” of coverage to choose from.

OPEN ENROLLMENT ACTION: If you would like to enroll in flexible spending for 2019, you MUST complete a new flexible spending application.
*Keep in mind that there is a roll over feature of up to $500.

We will also be adding an exciting new flexible spending benefit to cover qualified transportation benefits! With Qualified Transportation Benefit Accounts (QTBA’s) you can set aside money TAX FREE to help save on work related transportation and parking benefits! Employees can contribute a max of $265/month for work related transit and $265/month for parking expenses!
OPEN ENROLLMENT ACTION: If you would like to enroll in flexible spending TRANSIT for 2019, you MUST complete a new flexible spending application found in the 2019 Flex Transit Kit.
Axiom’s vision coverage is through United Healthcare, a leading vision provider that delivers affordable, innovative vision care solutions through a very diversified vision care network. You will have a choice of both private practice and retail chain providers.
OPEN ENROLLMENT ACTION: If you do not wish to make any changes to your vision insurance plan, you do not need to take any action at this time. If you would like to make a change to your vision insurance plan, or enroll if you are not already a participant, please complete the following form and return to HR::

For 2019 we will remain with Unum, offering one of the largest networks in the country and has more than 100,000 providers nationwide.
OPEN ENROLLMENT ACTION: If you do not wish to make any changes to your dental insurance plan, you do not need to take any action at this time. If you would like to make a change to your dental insurance plan, or enroll if you are not already a participant, please complete the following form and return to HR:

The 401k plan is through John Hancock, we will not be making any changes to the plan at this time. If you would like to enroll or change your 401K contribution percentage, you are able to do so at this time. If you would like to enroll in the plan you are able to do so online by following the attached instructions:

The 2019 pre-tax contribution limit for 401k plans will be $19,000. For those over the age of 50, the “catch-up” contribution limit will be $25,000.
Please contact Human Resources for more detailed plan summaries if needed! Forms can be scanned and e-mailed to , faxed to 703-205-2943, or dropped off to 2941 Fairview Park, Suite #900. If you do choose to e-mail any enrollment forms, please do not include your SSN. Please return all forms no later than December 14th.
