Seeking a Nurse Consultant to join Axiom Resource Management in support of the TRICARE Health Plan (THP) in the Defense Health Agency (DHA).
Successful Candidate will:
• Provide expertise by monitoring all aspects of Disease Management (DM) performance for care delivered primarily for the managed care support contractors (MCSCs) to include the six designated providers (DPs). Conduct overarching planning to support operations and TRICARE policy implementation.
• Work to ensure that the Department of Defense beneficiaries receive timely, appropriate in both scope and level, cost effective, quality, sensitive and coordinated care to affect desired clinical outcomes utilizing the major components of Population Health and DM.
• Participate in Population Based Health Care Management Services, applying and promoting DM and wellness/prevention concepts and strategies to the Primary Care assigned patients and those identified for DM.
• Conduct reviews and evaluate conflicts and programmatic issues to aid in resolving issues and to improve patient access, efficiency, and effectiveness of regional care.
• Liaison between the MCSCs and the direct care system (MTFs), you will be responsible for ensuring that DM activities are carried out in a way that enhances the services and quality of care to TRICARE beneficiaries.
• Participate and advise in the development of recommendations for DM regulations, guidelines, and procedures relating to program and business operations. Assist TRICARE in its efforts to improve patient outcomes and the efficiency and effectiveness of DM programs.
Desired or Preferred Requirements:
• Bachelor’s Degree in nursing is required, Master’s Degree in nursing is strongly preferred
• Registered Nurse or Nurse Practitioner license
• Minimum 10 years nursing experience
• Population Health and Disease Management experience is strongly desired
• Working knowledge of TRICARE policies and procedures is strongly desired
• Excellent written and verbal communication skills
• Able to work independently, in teams and manage time effectively
• Strong analytical abilities

• Experience participating in all phases of the SDLC, from requirements validation, through development, testing, and deployment.
